Methods 60 patients with dilated cardiomyopathy with an LVEF less than 40% were involved in this patients have received digoxin, ACE inhibito and diuretics for six months as the standard were randomly divided into two groups: carvedilol group(n=30) and controlled group(n=30). The patients of carvedilol group received an initial dosage of mg carvedilol twice daily for two weeks, and followed by increase at two-week intervals (if tolerated), fit to mg, then to mg, and, finally, to a target dosage of 25mg twice examinatio, radionuclide studies ,and determinatio of plasma levels of tumour necrosis factor-alpha (TNF-a),interleukin (IL-2、IL-6 ) were performed at baseline and repeated four months later. Results Patients treated with carvedilol showed a significant improvement in functional class compared with the baseline values (P=), and the LVEF of carvedilol group increased from ±% to ±% (P=), parallel with a decrease in the levels of cytokines (IL-6 [P=] and TNF-a[P=]). However,the controlled group showed no any improvement of LVEF and the level of inflammatory cytokines. Eight patients died (seven in the controlled group, P=).
